Association of severe mental illness with stroke outcomes and process-of-care quality indicators: nationwide cohort study

Background
Severe mental illness (SMI) is associated with increased stroke risk, but little is known about how SMI relates to stroke prognosis and receipt of acute care.
AimsTo determine the association between SMI and stroke outcomes and receipt of process-of-care quality indicators (such as timely admission to stroke unit).
MethodWe conducted a cohort study using routinely collected linked data-sets, including adults with a first hospital admission for stroke in Scotland during 1991–2014, with process-of-care quality indicator data available from 2010. We identified pre-existing schizophrenia, bipolar disorder and major depression from hospital records. We used logistic regression to evaluate 30-day, 1-year and 5-year mortality and receipt of process-of-care quality indicators by pre-existing SMI, adjusting for sociodemographic and clinical factors. We used Cox regression to evaluate further stroke and vascular events (stroke and myocardial infarction).
ResultsAmong 228 699 patients who had had a stroke, 1186 (0.5%), 859 (0.4%), 7308 (3.2%) had schizophrenia, bipolar disorder and major depression, respectively. Overall, median follow-up was 2.6 years. Compared with adults without a record of mental illness, 30-day mortality was higher for schizophrenia (adjusted odds ratio (aOR) = 1.33, 95% CI 1.16–1.52), bipolar disorder (aOR = 1.37, 95% CI 1.18–1.60) and major depression (aOR = 1.11, 95% CI 1.05–1.18). Each disorder was also associated with marked increased risk of 1-year and 5-year mortality and further stroke and vascular events. There were no clear differences in receipt of process-of-care quality indicators.
ConclusionsPre-existing SMI was associated with higher risks of mortality and further vascular events. Urgent action is needed to better understand and address the reasons for these disparities.

Background
Psychiatric disorders are associated with increased risk of ischaemic heart disease (IHD) and stroke, but it is not known whether the associations or the role of sociodemographic factors have changed over time.
AimsTo investigate the association between psychiatric disorders and IHD and stroke, by time period and sociodemographic factors.
MethodWe used Scottish population-based records from 1991 to 2015 to create retrospective cohorts with a hospital record for psychiatric disorders of interest (schizophrenia, bipolar disorder or depression) or no record of hospital admission for mental illness. We estimated incidence and relative risks of IHD and stroke in people with versus without psychiatric disorders by calendar year, age, gender and area-based deprivation level.
ResultsIn all cohorts, incidence of IHD (645 393 events) and stroke (276 073 events) decreased over time, but relative risks decreased for depression only. In 2015, at the mean age at event onset, relative risks were 2- to 2.5-fold higher in people with versus without a psychiatric disorder. Age at incidence of outcome differed by cohort, gender and socioeconomic status. Relative but not absolute risks were generally higher in women than men. Increasing deprivation conveys a greater absolute risk of IHD for people with bipolar disorder or depression.
ConclusionsDespite declines in absolute rates of IHD and stroke, relative risks remain high in those with versus without psychiatric disorders. Cardiovascular disease monitoring and prevention approaches may need to be tailored by psychiatric disorder and cardiovascular outcome, and be targeted, for example, by age and deprivation level.

Aim
The study purpose was to provide evidence of validity for the Primary Health Care Engagement (PHCE) Scale, based on exploratory factor analysis and reliability findings from a large national survey of regulated nurses residing and working in rural and remote Canadian communities.
BackgroundThere are currently no published provider-level instruments to adequately assess delivery of community-based primary health care, relevant to ongoing primary health care (PHC) reform strategies across Canada and elsewhere. The PHCE Scale reflects a contemporary approach that emphasizes community-oriented and community-based elements of PHC delivery.
MethodsData from the pan-Canadian Nursing Practice in Rural and Remote Canada II (RRNII) survey were used to conduct an exploratory factor analysis and evaluate the internal consistency reliability of the final PHCE Scale.
FindingsThe RRNII survey sample included 1587 registered nurses, nurse practitioners, licensed practical nurses, and registered psychiatric nurses residing and working in rural and remote Canada. Exploratory factor analysis identified an eight-factor structure across 28 items overall, and good internal consistency reliability was indicated by an α estimate of 0.89 for the final scale. The final 28-item PHCE Scale includes three of four elements in a contemporary approach to PHC (accessibility/availability, community participation, and intersectoral team) and most community-oriented/based elements of PHC (interdisciplinary collaboration, person-centred, continuity, population orientation, and quality improvement). We recommend additional psychometric testing in a range of health care providers and settings, as the PHCE Scale shows promise as a tool for health care planners and researchers to test interventions and track progress in primary health care reform.

Aim
To report the development and psychometric evaluation of a scale to measure rural and remote (rural/remote) nurses’ perceptions of the engagement of their workplaces in key dimensions of primary health care (PHC).
BackgroundAmidst ongoing PHC reforms, a comprehensive instrument is needed to evaluate the degree to which rural/remote health care settings are involved in the key dimensions that characterize PHC delivery, particularly from the perspective of professionals delivering care.
MethodsThis study followed a three-phase process of instrument development and psychometric evaluation. A literature review and expert consultation informed instrument development in the first phase, followed by an iterative process of content evaluation in the second phase. In the final phase, a pilot survey was undertaken and item discrimination analysis employed to evaluate the internal consistency reliability of each subscale in the preliminary 60-item Primary Health Care Engagement (PHCE) Scale. The 60-item scale was subsequently refined to a 40-item instrument.
FindingsThe pilot survey sample included 89 nurses in current practice who had experience in rural/remote practice settings. Participants completed either a web-based or paper survey from September to December, 2013. Following item discrimination analysis, the 60-item instrument was refined to a 40-item PHCE Scale consisting of 10 subscales, each including three to five items. Alpha estimates of the 10 refined subscales ranged from 0.61 to 0.83, with seven of the subscales demonstrating acceptable reliability (α⩾0.70). The refined 40-item instrument exhibited good internal consistency reliability (α=0.91). The 40-item PHCE Scale may be considered for use in future studies regardless of locale, to measure the extent to which health care professionals perceive their workplaces to be engaged in key dimensions of PHC.

We present a novel application for radiocarbon dating by aging 4 scales from a single large adult lungfish (Neoceratodus forsteri) from the Burnett River, in Queensland, Australia. The relict fish species lack a solid crystalline otolith, which precludes the use of the otolith annuli as a reliable age indicator. Previous attempts to age lungfish using a number of techniques have had only limited success. We report on ages obtained from the dense lamellar bone of the scale, which were isolated from the organic layers that thicken and subsequently obscure the 14C signal. Using the characteristics of the bomb curve, 2 parameter von Bertanalffy growth functions were fit, providing an estimate of absolute age to be ∼65–70 yr. The information gleaned from this study will aid in assessing the population structure, and therefore management, of this vulnerable species.

Introduction
The importance of males to the survival of infants is argued to be a major force in the evolution of male-female association in primates (van Schaik, 1996; Palombit et al., 1997; Sterck et al., 1997; Palombit, 1999). Across taxa, however, the extent and nature of males“ interactions with immatures, especially infants, vary enormously, from intense, directed caregiving in some NewWorld monkeys like marmoset, tamarins, and titis (Wright, 1984; Goldizen, 1987), to mere tolerance with occasional affiliation as in many Old World monkeys (Whitten, 1987; Maestripieri, 1998). Differences across and within species have been linked to various functions of males interactions with infants: while in some cases they represent investment by males in related infants, usually offspring (e.g. Busse and Hamilton, 1981; Anderson, 1992), in others, males appear to be using infants as social tools, either in their competitive interactions with other males (Paul et al., 1996), or as means to gain mating access to infants-mothers (Smuts, 1985; Ferrari, 1992; van Schaik & Paul, 1996). To add to the complexity, several functions might apply in the same species. In baboons, for example, all three have been invoked to explain male-infant interactions (Packer, 1980; Smuts & Gubernick, 1992).
In considering the function of male-immature relationships researchers have examined the benefits that immmature animals gain from their associations with males. It has been argued that, for most polygynous primates, the primary contribution that males make to their offspring's fitness is protection from predators and/or infanticide by unrelated adult males (Dunbar, 1984; van Schaik, 1996; Palombit, 1999; van Schaik & Janson, 2000).

Introduction
In January 1999, the Max Planck Institute for Evolutionary Anthropology hosted a conference in Leipzig, to celebrate more than three decades of research on wild mountain gorillas at the Karisoke Research Center. To be more specific, it was 32 years and 9 months since Dian Fossey had set up camp in the Rwandan sector of the Virunga Volcanoes (Figure 1.1). On September 24, 1967, using a marriage of Karisimbi and Visoke, the names of the two closest volcanoes, Fossey christened her site Karisoke. She could not have known that this would be her home for the rest of her life, or that the tent she pitched at 3000m in that wet, montane forest would become one of the longest-running research sites in field primatology. Many of the direct descendants of the mountain gorillas she first contacted in 1967 are still being observed today.
The story of Karisoke is a chronicle of the development of behavioral and ecological research, intertwined with the growth of conservation efforts to save mountain gorillas. It has been played out against a backdrop of political instability and, over the past decade, devastating war. We present briefly this story below, to set the stage for the chapters that follow (for a more detailed description of the development and history of behavioral ecology, we recommend Strier, 1994 and Janson, 2000).
The intellectual setting
By 1967, primatology and anthropology were ripe for a long-term study of gorillas. It was four years after the publication of George Schaller's classic work, The Mountain Gorilla, a landmark study of remarkable detail that described the basics of the subspecies‘ social organization, life history, and ecology (Schaller, 1963).

Introduction
Schaller (1963) and Fossey (1983) pioneered field study of gorillas, including study of the gorillas vocal communication (Schaller, 1963: ch. 5; Fossey, 1972). Their analyses were mostly of the males calls, indeed mostly of males loud, aggressive calls. We knownowthat gorillas give more or less benign within-group calls, close-calls, far more frequently than they roar, or scream, or bark (Harcourt et al., 1993). However, Fossey and Schaller were absolutely correct in identifying the long-calls and aggressive calls as frequent, and most often given by males – in the relatively unhabituated groups that they studied. Thanks to their lead, subsequent observers of gorillas could concentrate on the more subtle within-group behaviors, and do so with habituated groups.
Fossey and Schaller were both interested mainly in the social implications of the calling, the sociology of vocal communication. How did calls the behavior of other animals? Did they affect different animals differently? In this, they were unusual. Most analyses of non-vocal behaviors, grooming, proximity, aggression, had been sociological: how were the behaviors used to negotiate interactions, relationships, access to resources, and so on (Hinde, 1976, 1983). By contrast, the study of primates vocal communication has been far more concerned with its semantics, with discovering connections between the nature of a call and its referent in the environment, than with its social implications (Markl, 1985; Marler & Mitani, 1988). While broadcast communication has often been thoroughly analyzed, We knowmuch much less about that interesting realm of private – communication which is at the very heart of complex and highly organized animal societies (Markl, 1985: 174).
